How to Download & Edit a PDF Form

1. Download the PDF Form

  • Click on the link provided for the PDF form.
  • If the form opens in your web browser, look for a Download or Save icon (usually a downward arrow or floppy disk symbol).
  • Click this icon to save the PDF to a location on your computer, such as the Desktop or Documents folder.

2. Open the PDF Form

  • Navigate to the location where you saved the PDF.
  • Double-click the file to open it.
  • If the form doesn’t open in your browser, you might need a PDF reader like Adobe Acrobat Reader.

3. Fill Out the Form

  • Click on the fields within the PDF to type your information.
  • Use the Tab key to move between fields.
  • If you prefer, you can print it, write by hand, and then scan or take a photo of the completed form.

4. Save the Completed Form

  • After filling out the form, go to the File menu in your PDF reader.
  • Select Save or Save As to store the completed form on your computer.

We recommend printing the form, and keeping it somewhere that the emergency services and your emergency contacts or care workers can find it easily. You may also wish to email the completed PDF to trusted contacts.
